Text Box: NEW AGENCIES: We have new contracts with the following home care agencies: VNA of Butler, Concordia, Gallagher, AseraCare, and Nightingale. Our outstanding reputation for quality services brought these agencies to us. These agreements represent new opportunities for our therapists who work in home care. NEW OASIS-C Therapists are doing an excellent job with the new forms. Please remember to contact the agency if you have questions. Many agencies are providing a tracking sheet with information from the Oasis “open” in the patient chart to assist you with completing the form. VACATION REQUESTS As you make your plans for vacation, whether you are full time or part time, please send your written request for time off as soon as possible. This will allow plenty of time for arranging coverage. CHART REVIEWS Please note that our own Cindy Thomas does independent chart reviews for many of our agencies. She evaluates the chart and provides feedback to agencies & therapists. This also ensures that our high standard for compliant documentation Text Box: DME ORDERING Patients are to be given the opportunity to choose a DME company. If the patient does not have a preference, the agency DME company should be utilized. If the agency does not have a DME company, there is a new and easy procedure for ordering pre-fab splints and DME for patients. We are coordinating this process through Elizur Corporation. Upon evaluation, if an orthotic or equipment is indicated: contact Renee Mead, OTR/L of Elizur at 412-913-9927. Fax her the patient face sheet and insurance information @ 412-348-1286 Renee will secure the order if necessary and bill insurance for the equipment. She will also coordinate with the therapist & patient a visit to measure and fit the splint . Upon fitting, another visit may be necessary to ensure proper fit and wear.
